1. Information we collect
- Contact details — name, phone number, email address you provide via our booking form, contact form, web chat, or SMS reply.
- Appointment information — date, time, service requested, practitioner, and any notes you choose to include.
- Communication content — SMS messages, chat conversations with our AI front desk (Jasmine), and voicemail transcripts.
- Health information — only what you choose to share with us in the context of seeking care (e.g., chief complaint described in the booking form).
- Technical data — IP address, browser, device type, pages visited, and timestamps, used for analytics and security.
2. How we use your information
- Schedule, confirm, remind, and reschedule appointments.
- Reply to your inquiries via SMS, email, voice, or web chat.
- Provide care during your visit and maintain clinical records as required by law.
- Send service updates and follow-ups directly related to your care.
- Comply with legal, regulatory, and licensing requirements.
We do not sell your personal information. We do not share your information with third parties for their own marketing purposes.
3. SMS messages
SMS is optional. Providing your phone number does not opt you in. You receive SMS from us only if you check the optional, unchecked-by-default SMS consent checkbox on our appointment request form. SMS consent is never required to submit a form, book an appointment, or receive care. Once opted in, you may receive appointment-related notifications and replies to your inquiries. Message frequency varies. Message and data rates may apply.

4. Service providers
We share information with vendors who process data on our behalf under written agreements that require confidentiality. These include the Bloma platform (website, AI front desk, SMS routing) and the underlying providers Bloma uses, such as Twilio (SMS & voice carrier), Anthropic (AI language model used by Jasmine), Vapi (voice infrastructure), Supabase (database), and Vercel (hosting).
5. Health information & HIPAA
We handle protected health information in accordance with applicable law. If you become a patient of our clinic, you will receive our Notice of Privacy Practices describing how we use and disclose protected health information for treatment, payment, and healthcare operations. The Bloma platform operates as a Business Associate where applicable.
6. Data retention
We retain communication records and booking data for as long as needed to provide care, comply with legal obligations, and resolve disputes. Clinical records are retained for the period required by state law. You may request deletion of non-clinical records by contacting us at the information below.

8. Children
Our services are not directed at children under 13. If a parent or guardian books on behalf of a minor patient, the booking is treated as the parent or guardian's booking.
